| ABSTRACT |
|
Since George Winter reported the usefulness of moist environment wound healing in 1962, the conceptual basis of wound healing has changed from dry dressing to moist wound dressing and effectiveness has been proven by numerous clinical applications. According to these conceptual basis, polyurethane foam, hydrocolloid, hydrogel sheet and hydrofiber has been used for dressing. Among them we evaluated the effect of hydrogel sheet in burn and abrasion wound. We clinically applied CligelⰒ (Hugel, Sungnam, Korea), which is one type of Hydrogel sheet, to 12 abrasion patients and 32 burn patients who visited Armed Forces Yang-Ju hospital. Treated abrasion patients who was not injured at their hair follicle took 8.3⁑2.1 days and patients already composing the eschar took 10.9⁑1.5 days to be healed. For burn patients, who had superficial second degree burn, it took 7.3⁑1.6 days and patients with deep second degree burn needed 12.9⁑1.6 days of healing period. Hydrogel sheet prevented eschar formation, therefore eschorotomy was of no need. Because of the structure of polyurethane foam, it has good contacting ability with body and absorbing ability. It is very useful for initial wound healing process because in this process much bleeding and exudation could be found. However, because of those abilities it could bring some pain in some healing process, especially in epithelialization state when changing the dressing material. Especially, with infants, who have weak skin structure, the dressing times are more limited. For burn patients, it's use is limited for first degree burn patient and after eschar composition, it's use is meaningless. On the other hand, the CligelⰒ is somewhat uncomfortable in inflammation and hemostasis phase of beginning healing process. But in the epithelialization phase, it has less pain and stimulates the epithelialization. Also it assists the functional dressing material like EasyefⰒ (Daewoong, Sungnam, Korea) and ActicoatⰒ (Smith & Nephew, Hull, England) or Aquacel AgⰒ (Convatec, New Jersey, USA) to moisture supply. Conclusively, the CligelⰒ is very useful for the abrasion and burn patients in reducing pain and promoting epithelialization. (J Korean Wound Care Soc 2005;1:52-56) |
